Job summary

Trinity Health Of New England is seeking an Oncology Social Worker (LCSW) for the Outpatient Oncology & Infusion Center. The role delivers psychosocial assessments, counseling, advocacy, and care coordination to patients and families across diagnosis, treatment, survivorship, and end-of-life care in a multidisciplinary cancer care setting.

Responsibilities include distress screenings, counseling, discharge planning, and resource coordination.

Qualifications

• MSW required from CSWE-accredited program.
• Connecticut Social Work licensure required; LCSW preferred; LMSW considered based on experience.
• Minimum of two years of social work experience in healthcare/oncology or related setting.

Responsibilities

• Conduct psychosocial assessments and distress screenings for oncology and infusion patients and families.
• Provide individual and family counseling related to cancer diagnosis, treatment, survivorship, and adjustment.
• Coordinate outpatient care transition and discharge planning based on patient needs and resources.
• Collaborate with physicians, nurses, navigators and other healthcare professionals for patient-centered care plans.
• Assist patients with access to financial assistance, insurance resources, transportation and community services.
• Coordinate referrals to behavioral health, hospice and palliative care as appropriate.
• Document assessments, interventions, and outcomes in the EMR.
• Participate in multidisciplinary case conferences and cancer program committees.
