Trinity Health Of New England is seeking an Oncology Social Worker (LCSW) for our outpatient oncology and infusion center in Hartford, CT. Qualifications

• MSW from CSWE-accredited program required.
• Connecticut Social Work licensure required.
• 2+ years healthcare/oncology social work experience preferred.
• Knowledge of psychosocial oncology and patient advocacy.
• Strong communication and interdisciplinary collaboration skills.

Responsibilities

• Conduct psychosocial assessments, distress screenings, and evaluations for oncology patients and families.
• Provide individual, family, and supportive counseling related to cancer diagnosis and survivorship.
• Coordinate outpatient care transition and discharge plans based on patient needs.
• Collaborate with physicians, nurses, navigators, and other professionals on care plans.
• Assist patients with access to financial resources, insurance, transportation, and support programs.
• Coordinate referrals to behavioral health, hospice, and community resources.
• Support survivorship care plan development and documentation in EHR.
• Participate in case conferences, quality initiatives, and cancer program committees.
• Advocate for patients while upholding confidentiality and ethical practice.
