Job summary

SQRL in Los Angeles, CA is seeking an Oncology Research Scribe to support investigators during patient visits. You will capture real-time documentation in the EMR and study source to help clinicians maintain protocol compliance and efficient patient throughput.

Responsibilities include mapping HPI, ROS, exam findings, assessments and plans in real time, transcribing physician dictation, routing follow-up orders, and coordinating with study staff while upholding HIPAA confidentiality.

Qualifications

• 1+ year of experience as a medical scribe.
• Knowledge of medical terminology, EMR systems, and clinical documentation standards.
• Ability to work on site in Century City area of Los Angeles, CA.

Responsibilities

• Accompany on-site during patient encounters to document history of present illness, review of systems, exam findings, assessment and plan in real time in the EMR.
• Transcribe physician dictation and observed clinical findings into structured EMR fields and study source templates.
• Enter and route follow-up orders (labs, imaging, referrals, scheduling) as directed by the physician.
• Flag incomplete or ambiguous documentation for physician clarification before finalizing notes.
• Maintain confidentiality of patient information per HIPAA and site policy.
• Coordinate with clinical research coordinators to ensure documentation aligns with protocol.
• Track and close out same-day follow-up items to prevent backlog.

The Role

The Oncology Research Scribe works alongside research oncologists and study staff during patient visits to capture accurate, timely clinical documentation in the EMR and study source. This role serves as a key support function, enabling physicians to focus on direct patient care. By ensuring visit notes, orders, and follow-up items are captured completely and correctly the first time, the Scribe directly supports faster patient throughput, stronger protocol compliance, and a smoother experience for patients considering or enrolled in a clinical trial.

• Accurately transcribe physician dictation and observed clinical findings into structured EMR fields, study source, and templates.
• Enter and route follow-up orders (labs, imaging, referrals, scheduling) as directed by the physician during or immediately after the visit.
• Flag incomplete or ambiguous documentation to the physician for clarification before the note is finalized.
• Maintain strict confidentiality of patient health information in accordance with HIPAA and site policy.
• Coordinate with clinical research coordinators to ensure visit documentation aligns with protocol-required source data.
• Track and close out same-day follow-up items (results review, callbacks, prior authorizations) to prevent documentation backlog.
