Southcoast Health is seeking an RN-Practice Navigator-Oncology for the Fairhaven Cancer Center in Fairhaven, MA. This 32-hour/day role focuses on care coordination across the cancer care continuum, supporting patients from diagnosis through survivorship.

Qualified candidates have a BSN, MA RN licensure, strong oncology background, and willingness to obtain OCN within 1 year. The position emphasizes collaboration, education, and access to community resources.

Qualifications

• Strong oncology background required.
• Certification like OCN or willingness to obtain within 1 year of hire.
• BSN or equivalent experience required.
• Oncology Patient Navigator training preferred.
• Basic knowledge of insurance reimbursement systems and financial hardships.
• Critical thinking and interpersonal skills.
• Ability to develop collaborative relationships internally and externally.
• Ability to multi-task, handle stressful situations in a fast-paced environment.
• Knowledge of community services available to patients and their families.
• Over three years of clinical oncology experience in an outpatient or inpatient setting.
• Current RN licensure in Massachusetts; CPR preferred.
• Oncology Nurse Certification preferred; within 18 months of employment.
• Must be fully vaccinated against influenza and COVID-19 or exempt via signed statement.
• Compensation: Pay rate will be determined based on level of experience.

Responsibilities

• Provide consistent care coordination across the continuum, assessing physical, psychological and social needs.
• Serve as ongoing resource for patients during initial diagnosis, treatment and survivorship.
• Organize and facilitate clinical visit needs.
• Educate patients and families and provide referrals to resources for cancer care.
• Support patient and families to ensure compliance with care.
