Responsibilities

• Perform comprehensive patient health assessments by interpreting data and trends.
• Formulate individualized care plans based on assessments and population data.
• Delivers patient- and family-centered care across the continuum of settings.
• Advocate for patients’ rights and provide guidance to resolve issues.
• Coordinate care with the healthcare team and arrange post-discharge services.
• Document all patient care activities per standards and support team members’ documentation.
