Job summary

Qualifications

Responsibilities

• Provides ongoing care (assess, plan, intervention, evaluation) according to age-specific needs
• Communicates with physicians to update plan of care and provide status updates
• Maintains accurate clinical and patient records
• Provides telephone consultation/triage per protocol
• Participates in community education and quality improvement activities
• Supports staff orientation and education activities
• Follows agency policies and monitors changes
• Maintains knowledge and practice in specialty area, including certifications as warranted
