Oncology RN Care Navigator - Triage & Coordination

Job summary

Responsibilities include triage of calls, handling medication orders (oral chemotherapy), coordinating patient care documentation, and providing nursing support on the care team. Oncologic infusion experience is preferred.

Qualifications

• Active RN license in Louisiana is required.
• BLS CPR certification required.
• ONS/OCN certification preferred for oncology practice.
• Phone triage experience is required.
• Oncology/Infusion nursing experience is preferred.

Responsibilities

• Triage patient calls and coordinate care for all physicians.
• Review all incoming faxes to the office.
• Answer pharmacy calls and medication refill requests.
• Coordinate physician orders (home health, hospice, hospital, etc.).
• Handle emergency calls on the designated emergency line.
• Serve as liaison between physician, facilities, and patient.
• Triage patient calls.
• Manage oral chemotherapy and medication orders.
• Coordinate patient care and document appropriately.
• Perform nursing duties as required.
• Serve as a resource for all staff.
