Job summary

Concierge Home Care is seeking a Home Hospital Oncology Registered Nurse (RN) in Jacksonville, FL. This role delivers high-acuity nursing care to oncology patients in their homes, focusing on treatment adherence, recovery, and patient education.

You’ll work with an HQ that emphasizes technology and clinical leadership. The position offers flexible scheduling, a strong onboarding program, and ongoing mentorship.

Qualifications

• Active Florida RN license or compact license
• Valid driver's license and reliable transportation
• Home health experience preferred but not required

Responsibilities

• Conduct patient assessments and coordinate care plans
• Administer oncology treatments and medications
• Perform port access and IV therapy/maintenance
• Perform lab draws and medication administration
• Monitor patient conditions and update physicians
• Educate patients and caregivers on treatment plans

Your Role as a Home Hospital Oncology Registered Nurse

As a Home Hospital Oncology RN, you will provide higher-acuity skilled nursing care to oncology patients in the comfort of their homes while supporting recovery, treatment adherence, and patient education.

This specialized Hospital-at-Home division does not require Medicare OASIS documentation, allowing you to focus more on direct patient care and clinical treatments.

Responsibilities include:

• Conduct patient assessments and coordinate care plans
• Administer specialized oncology treatments and medications
• Perform port access and IV therapy/maintenance
• Perform lab draws and medication administration
• Monitor patient conditions and communicate updates to physicians
• Educate patients and caregivers on treatment plans and symptom management
• Support post-surgical recovery and treatment adherence
• Collaborate with physicians and interdisciplinary care teams
• Document visits efficiently using AI-assisted EMR tools
