The candidate will monitor therapies, provide drug information, and contribute to education and research programs across service lines within the hospital's oncology services.

Qualifications

• Strong communication and documentation skills are required.
• Experience with pharmacy information systems and general computer applications.
• Interest/knowledge in pathophysiology and pharmacotherapy for oncology.
• Minimum 1 year hospital pharmacy experience; or 2 years hospital experience or PGY1/general practice residency.

Responsibilities

• Provide clinical services and medication therapy management for oncology patients.
• Serve as a primary preceptor for pharmacy students and residents.
• Collaborate with multidisciplinary teams to optimize patient care and safety.
• Monitor drug therapies for interactions, contraindications, and appropriate dosing.
• Participate in formulary management and cost-effective therapeutic substitutions.
