UC Davis Health is seeking a Supportive Oncology Patient Navigator to help adult cancer patients access resources, coordinate care, and overcome barriers in outpatient settings. Under supervision, you will assess needs and educate families while coordinating with supportive services to improve adherence and experience.

The role supports the UC Davis Comprehensive Cancer Center within the Supportive Oncology Services, and requires strong communication, time management, and collaboration in a 100%

Job Summary

The role of the Supportive Oncology Patient Navigator is to provide support, education, referrals, and resources to help navigate adult cancer patients throughout their care within the outpatient adult oncology care settings. Under general direction and team collaboration, the Supportive Oncology Patient Navigator conducts needs assessments, identifies barriers to care, coordinates efforts to reduce barriers by connecting patients to UCDH and community services, facilitates logistical needs to improve treatment adherence, and provides education, guidance and support to address supportive care needs. #CA- SB

• High School graduation or GED equivalent and/or equivalent experience/training
• Experience working with the general public and community agencies in collaborative and professional settings.
• Experience with multi-cultural, multi-generational families, including non-traditional family structures.
• Strong customer service skills and cultural humility orientation.
• Skills related to multi-disciplinary team, able to promote and engage in relationship building with healthcare teams, patients and caregivers.
• Strong professional boundaries, able to work collaboratively utilizing a team approach.
• Excellent communication skills, including positive interpersonal skills working with diverse people.
• Proficient computer skills, including Microsoft Word, Excel, Power point.
• Ability to curate computer searches and distinguish safe valid and vetted information from reliable sources.
• Efficient with time management and working autonomously as needed.
• Attention to detail and timely delivery of accurate and complete work.
• Ability to organize and prioritize multiple tasks according to patient safety and program needs.

Preferred Qualifications

• Bachelor's degree, preferably in the fields of social work (BSW), psychology, health care or human services
• Experience working in a health care setting.
• Experience serving cancer patients and their families.
• Experience navigating community resources and linking patients and families to services to assist them.
• Bilingual fluency - Spanish, Russian, Tagalog or Vietnamese.
• Familiarity with oncology knowledge, cancer-related psychosocial impact.
• Knowledge of Medicaid/Medi-Cal and educating patients/clients on their benefits and how to use them.

Department Overview

UC Davis Health Supportive Oncology Services (SOS) is dedicated to serving individuals whose lives are impacted by cancer through comprehensive assessment and expert management of the complex challenges associated with their care. As a member of the SOS team, the Supportive Oncology Patient Navigator provides supportive care services to patients and families at the UC Davis Comprehensive Cancer Center, helping to facilitate access to resources, coordinate care, and enhance the overall patient experience.

Department Specific Job Scope

This position serves the UC Davis Comprehensive Cancer Center, a division or Ambulatory Care Services and is under the Supportive Oncology Department. The position serves as a non-clinical resource and liaison for patients and their families. Assists with navigating all aspects of the healthcare system. Uses knowledge of the clinic to resolve issues and identify ways to increase patient satisfaction. Collaborates with department manager to meet patient expectations, achieve quality outcomes and build a patient focused culture.
