Chesapeake Urology is seeking a Medical Oncology Nurse Navigator (MONN) to guide patients and families through the cancer care continuum. You will collaborate with scheduling, financial counseling, pharmacy, lab, social work, and clinical teams to ensure timely access to high-quality care.

The MONN educates, facilitates decision-making, and connects patients with resources while supporting survivorship and care planning. Comprehensive benefits and a patient‑centered approach are offered.

Formación

• Bachelor’s prepared Registered Nurse (BSN) required.
• Oncology Certification (OCN) preferred, Nurse Navigator Certification required to be completed within first year of employment.
• Five (5) years work experience as a Registered Nurse in an oncology practice or clinic
• Urology or surgical experience preferred but not essential.

Responsabilidades

• Coordinates care across the cancer care continuum and supports navigation of patients through the care pathway.
• Identifies gaps in care and coordinates interventions to close them.
• Organizes and participates in Patient Care Conferences, communicating outcomes to the care team.
• Performs risk assessments and provides psychosocial support, referring for further screening as needed.
• Provides education on oral chemotherapy, adherence, and side effects.
• Holds survivorship and advance care planning visits; makes referrals as needed.
• Participates in research, quality, and performance improvement, including reporting navigation metrics.
• Supports patient advocacy and community cancer prevention education.
