Jupiter Medical Center in Palm Beach County seeks a Nurse Navigator to serve as the single point of contact for referring physicians, patients, and caregivers, helping to overcome treatment barriers and expedite access to clinical and supportive services.

The role includes maintaining updated treatment plans, assisting with appointments, labs, and specialty visits, and participating in community outreach to promote the program and Jupiter Medical Center system.

Qualifications

• Current RN license in Florida.
• American Heart Association BLS certification.
• Five years of nursing experience with three in oncology preferred.
• Knowledge of physician office processes.
• Excellent verbal and written skills.
• Proficient in building relationships and coordination of care.
• Knowledgeable about oncology disease processes and symptom management.
• Ability to foster and maintain effective working relationships across patients, staff, physicians and administration.

Responsabilités

• Provide single point of contact for refer­ring physicians, patients and caregivers to access resources.
• Assist in overcoming treatment barriers and expediting access to clinical and supportive services.
• Prepare and maintain updated treatment plans to synchronize care management.
• Assist with patient appointments, labs, diagnostics and specialty visits.
• Engage in community outreach to promote the program and Jupiter Medical Center system.
• Perform other duties as assigned.
