Jupiter Medical Center seeks a Nurse Navigator to act as a single point of contact for referring physicians, patients, and caregivers, helping to overcome treatment barriers and expedite access to care services provided within the Center and community.

The Nurse Navigator will prepare and maintain updated treatment plans, assist with appointments, and participate in community outreach to promote the program.

Qualifications

• RN licensure in Florida is required.
• Oncology experience is preferred.
• BLS Certification for Healthcare Providers is required.
• Must be able to coordinate care and communicate effectively.
• Strong relationship-building and leadership skills are essential.

Responsabilités

• Serve as a single point of contact for referring physicians, patients, and caregivers.
• Prepare and maintain updated treatment plans to synchronize care management.
• Assist with patient appointments, including labs and specialty visits.
• Engage in community outreach to promote the program and Jupiter Medical Center.
