Job summary

Responsibilities

• Coordinate multiple aspects for cancer patients within Reid Health System.
• Provide continuity of care with education, care coordination and psychosocial support.
• Work with service line leadership to develop educational programs, policies and procedures, and goal setting.
• Engage in problem solving to improve patient care in a complex healthcare environment.
• Assess and plan patient care, maintaining a plan across the continuum with patient, family and health team.

Education

Bachelor of Science in Nursing

Master’s Degree in Nursing (preferred)
