Qualifications

Responsibilities

• Facilitates patient needs assessment and coordination of care with multidisciplinary teams
• Schedules appointments, testing, and procedures to expedite care
• Identifies barriers to care and facilitates referrals
• Educates patients and caregivers on disease guidelines and treatment options
• Performs data collection for service line reporting
