Odessa Regional Medical Center is seeking a part-time RN Patient Navigator for a Clinic Population Health Nurse role. The position involves coordinating care with physicians and hospital resources, managing high-risk patients, and supporting preventive care across settings in Odessa, TX.

The role requires an ADN with Texas RN licensure (BSN preferred), BLS certification, and 3 years of related experience. On-site, with a part-time schedule, serving in a population health capacity.

Qualifications

• ADN required; BSN preferred.
• RN license in Texas required; maintain licensure in good standing.
• 3 years of experience in patient education, nursing, planning, case management, or similar.
• Current active, unrestricted RN license in the State of Texas.
• AHA BLS certification required; other certifications preferred.

Responsabilités

• Manage resources across interdisciplinary teams to achieve optimal patient outcomes.
• Coordinate data, physicians, and hospital resources to create seamless access to care.
• Identify and manage high-risk patients through assessment and care coordination.
• Monitor registries and reports to identify care gaps and interventions.
• Coordinate care transitions after hospital encounters; follow up with patients.
