Job summary

Endeavor Health is seeking a Clinical Pharmacist to provide comprehensive pharmaceutical care to cancer patients at Swedish Hospital in Chicago, with rotation to Arlington Heights. The position is per diem and requires willingness to travel to Kellogg Cancer Center sites as needed.

Hours are variable, including weekends and holidays. Applicants must hold a current Illinois Pharmacist license, have a BS or PharmD, and ideally board certification in oncology or equivalent experience, with at least

Qualifications

• Current Illinois Registered Pharmacist.
• BS or PharmD from an accredited school of pharmacy (preferred).
• Board certification in oncology training or equivalent experience preferred.

Responsibilities

• Provide and document comprehensive pharmaceutical care to NorthShore patients with cancer and related conditions.
• Provide medication education and symptom management to oncology patients and caregivers.
• Review and process chemotherapy and supportive care orders for clinical appropriateness.
• Prepare and dispense anti-cancer and supportive care medications following best practices.
• Design or update chemotherapy plans and monitoring care plans for oncology patients.
• Provide consultative services to other health care professionals on anti-cancer drug therapy and monitoring.
