Job summary

Endeavor Health is seeking a Clinical Pharmacist to join our oncology team at Swedish Hospital in Chicago and at the Arlington Heights site. This per diem position requires an Illinois Pharmacist license, 3+ years of clinical pharmacy experience, and oncology training or equivalent experience.

Responsibilities include providing patient-centered pharmaceutical care, educating patients and caregivers, reviewing chemotherapy orders, preparing anti-cancer medications, and contributing to care plans.

Qualifications

• License: Current Illinois Registered Pharmacist
• Education: BS or PharmD (preferred)
• Certification: Board certification in oncology training or equivalent experience preferred
• Experience: At least 3 years of experience as a clinical registered pharmacist

Responsibilities

• Provide and document comprehensive pharmaceutical care to NorthShore patients with cancer and other conditions related to cancer
• Provide and document medication education and symptom management to oncology patients and caregivers
• Review and process chemotherapy and supportive care orders plans for clinical appropriateness
• Prepare and dispense anti-cancer and supportive care medications for oncology patients following best practices and the organization's policies and procedures
• Design or update chemotherapy plans, supportive care regimens and monitoring plans (care plans) for oncology patients
• Design effective educational materials for patients on chemotherapy
• Provide chemotherapy infusion reaction management to oncology patients
• Provide consultative services to other health care professionals on anti-cancer drug therapy and monitoring
• Provide medication education to pharmacists and other health care professionals
• Precept learners (PGY2 oncology resident, PGY1 residents, and students)
