UCLA Health in Atascadero, CA is seeking a pharmacist with a focus in oncology to support medication management for oncology patients. You will maintain and review patient medication profiles, verify orders, and ensure safe IV product use while collaborating with the care team.

The role requires a PharmD, oncology residency or equivalent experience, and travel to multiple UCLA Health sites. Strong communication and pharmacokinetic skills are essential; BCOP is preferred.

Qualificações

• PharmD required.
• ASHP PGY2 Oncology or 1+ year oncology pharmacy experience.
• Recent oncology experience (within 5 years).
• BCOP certification preferred.
• Infusion pharmacy experience preferred.
• Excellent communication and organizational skills.
• Ability to travel to multiple UCLA Health sites.

Responsabilidades

• Maintain and review patient medication profiles.
• Review and verify physician orders and patient records.
• Check IV products for accuracy and safety.
• Collaborate with healthcare teams to promote safe medication use.
• Provide clinical pharmacy services (pharmacokinetics, IV/PO conversions, therapeutic interchange).
