Providence Health Plan Group seeks a Nurse Navigator in Infusion to coordinate and administer cancer treatment regimens, research components, and program outreach. You will support patients and families, advocate for whole-person health, and tailor services to wellness needs while building strong relationships with the care team.
The role emphasizes direct patient care, data collection on referrals, and alignment with Providence's mission and values.
Qualificações
- Bachelor’s degree in nursing required.
- California RN license required upon hire.
- National Provider BLS certification within 30 days.
- Oncology certifications or completion within 1 year of hire.
- ONS chemotherapy administration card required upon hire.
- 5+ years RN experience in healthcare.
- 3+ years oncology nursing experience.
Responsabilidades
- Coordinate and administer cancer treatment regimens and related care.
- Provide patient education and support for patients and families.
- Cultivate relationships with patients, families and care partners.
- Collect data on contacts/referrals and manage care pathways.
- Advocate for whole-person health aligned with Providence mission.
