Job summary

Norwalk Hospital Association is seeking a Clinical Pharmacy Specialist in Oncology Pharmacy to serve as the lead clinical pharmacist within their area of specialty. The role includes governance, teaching, education, and developing new workflows to optimize pharmacotherapy for oncology patients.

The position requires pharmacy licensure in NY or CT, a PharmD/BS with advanced credentials, and substantial inpatient hospital experience.

Qualifications

• Required: BS of Pharmacy or PharmD.
• Minimum 5 years inpatient hospital experience; PGY2 Residency; or 2 years inpatient hospital experience in addition to PGY1.
• Active Pharmacist license in applicable state (NY or CT).
• Board Certification/Advanced degree required (within one year of hire) or experience within specialty area.

Responsibilities

• Oversee/Check/Monitor Central Pharmacy Workflow: oversee drug preparation, dispensing and distribution; supervise technicians; ensure regulatory compliance.
• Clinical Pharmacy and Drug Information: maintain drug knowledge; provide drug information to the care team; perform daily clinical functions (IV to PO conversions, dosing, renal adjustments).
• Patient Counseling and Communication: counsel patients on therapy; communicate with medical team; present in multidisciplinary settings.
