Job summary

City of Hope seeks a Clinical Pharmacist Specialist to join their team in Duarte, California. The specialist will work autonomously to provide evidence-based pharmaceutical care, collaborate with healthcare teams, and ensure optimal medication management.

This role requires a graduate of an ACPE-accredited Pharmacy School, a current CA Pharmacist License, and relevant residency or experience. Responsibilities include participating in team rounds, evaluating medication orders, and educating staff about oncology therapies.

Qualifications

• Graduate of an ACPE-accredited Pharmacy School.
• Pharmacy Practice Residency (PGY 1) and Specialty Residency (PGY 2) or equivalent experience.
• Current CA Pharmacist License required.
• BLS certification required.

Responsibilities

• Participate in multidisciplinary team rounds.
• Evaluate medication orders for appropriateness.
• Develop and implement drug therapy plans.
• Monitor treatments and provide follow-up recommendations.
• Educate staff on oncology therapies.
