Job summary

City of Hope’s Clinical Pharmacist II will deliver high-level clinical care as part of a multidisciplinary team in the Day Hospital, focusing on patients undergoing outpatient allogeneic hematopoietic cell transplantation. You will lead medication management and coordinate care with oncology services to ensure optimal patient outcomes.

Responsibilities include rounds, education for patients and families, development of order sets, and participation in research and quality initiatives.

Qualifications

  • Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending PGY1 and PGY2 residencies, or PGY1 plus two years of experience in an acute care hospital pharmacy, or Five years of experience in an acute care hospital pharmacy.
  • Current CA Pharmacist License.
  • BLS certification through the American Heart Association.

  • Participates in daily clinical care rounds and interdisciplinary team meetings.
  • Reviews medication regimens for safety, interactions and contraindications.

The Clinical Pharmacist II plays a key role in providing high-level clinical care as part of a multidisciplinary healthcare team, focusing on direct patient care. In this role, the inpatient hospital clinical pharmacist takes ownership of workflows and processes within their area of expertise and actively contributes to the design, implementation, documentation, and monitoring of comprehensive medication management (CMM) and therapeutic care plans. Through close collaboration with patients and providers, they ensure positive, patient-centered outcomes. Serving as a vital link between Pharmacy Services and Hospital & Specialty Oncology Services, the clinical pharmacist plays a crucial role in facilitating discharges and coordinating care to enhance continuity and improve patient outcomes. This Clinical Pharmacist II position will be based at Day Hospital to care for patients undergoing outpatient allogeneic hematopoietic cell transplantation (HCT). The pharmacist will be overseeing these transplant patients and round with the transplant team. The position will also help support our Hospitalist (HP) service.

As a successful candidate, you will:
  • Provides comprehensive medication management services, coordinating all clinical pharmacy functions for the acute care areas as assigned to help achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.
  • Completes patient profile reviews and participates in daily clinical care rounds, huddles, and meetings as an integrated member of a multidisciplinary team.
  • Reviews medication regimens for appropriateness, contraindications, and potential interactions.
  • Works directly with patients, families, and caregivers to provide comprehensive education about medications and related disease state(s). Verifies patient understanding of the medication including its intended purpose, proper use, expected outcomes, and how to obtain refills/renewals. Ensures an updated medication profile and list of medications are provided.
  • Collaborates with pharmacy supervisors, leadership, and staff to develop policies, procedures, and processes for more efficient processing within the pharmacy Enterprise and area of practice.
  • Works with multidisciplinary teams to develop, implement, evaluate, and update order sets, Treatment and Supportive Plans to promote best practice care.
  • Ensures compliance with medication management standards outlined by The Joint Commission (TJC), COH, and other accrediting bodies.
  • Exhibits expert understanding of regulatory and quality standards for their program area.
  • Serves as a liaison between Pharmacy Service and Hospital and Specialty Oncology Services staff.
  • Develops an understanding of therapeutic clinical studies in their areas of expertise and manage patients’ treatment based on study requirements and specifications.
  • Participates in Oncology research and contribute to cancer discoveries and the clinical trials being conducted by investigators and doctors at City of Hope.
  • Assists in designing, developing, conducting, and presenting the results of Medication Use Evaluations (MUEs), and other projects related to patient outcomes.
  • Participates as a primary preceptor for pharmacy resident projects.
  • Performs all other duties as assigned by pharmacy leadership.
