Job summary

Trinity Health Of New England is seeking a Clinical Staff Pharmacist to provide patient-centered pharmacy care within a hospital setting and to serve as a clinical leader to students, residents, and staff. This role emphasizes rounding, medication therapy plans, and collaboration with the interdisciplinary team.

You will precept IPPE/APPE rotations, supervise technicians, verify orders, monitor therapies, and participate in program development.

Qualifications

• BS degree in Pharmacy; PharmD degree preferred.
• Hospital experience and/or Residency preferred.
• CT Pharmacist license.
• Competency in providing direct patient care across age groups (neonate to geriatric).

Responsibilities

• Supervise non-pharmacist personnel by assigning duties and verifying orders.
• Assess need for non-formulary items and coordinate substitutions with physicians.
• Verify orders for dose, allergies, interactions, ADRs; monitor labs and communicate issues.
• Maintain narcotics inventory; conduct weekly/monthly inventories and proper filing.
• Advise clinicians on drug interactions, incompatibilities, side effects, and administration.
• Actively participate in department programs and projects.
