Job summary

Qualifications

• Pharmacist must hold a PharmD or BS in Pharmacy from an accredited college.
• PGY2 residency in oncology preferred; or PGY1 with 2 years oncology/clincal specialist experience or 5 years oncology experience.
• Massachusetts pharmacist license is required; board certification in specialty within 18 months of employment.

Responsibilities

• Practices as a pharmacist following all applicable regulations and standards.
• Reviews medication orders for appropriateness and completeness based on patient data.
• Provides clinical consultation and evidence-based recommendations for pharmacotherapy to clinicians.
• Collaborates with interdisciplinary team during rounds, huddles, and case conferences.
• Educates patients and caregivers on oncology therapies and maintains education materials.
• Documents care activities in the EHR and communicates changes to the care team.
• Participates in scholarly and educational activities as required.
• Involved in policy development, workflows, and program initiatives.
• Performs other duties as assigned.
