Job summary

Dana-Farber Cancer Institute in Boston is seeking a Clinical Services Pharmacist III to deliver clinical pharmacy services for outpatient oncology patients. You will collaborate with physicians, nurses, and the pharmacy team to optimize antineoplastic therapies and patient care.

The role includes counseling patients starting IV and oral anticancer therapy, conducting clinical assessments, monitoring adherence and toxicity, and reviewing orders prior to infusion.

Qualifications

• Pharmacists require a PharmD or PharmB degree from an accredited college.
• Residency in oncology or related clinical experience preferred.
• Board certification in their specialty area is desirable.

Responsibilities

• Provide pharmacy services for outpatient oncology patients as part of the clinical team.
• Counsel patients starting intravenous and/or oral antineoplastics.
• Perform clinical assessments, adherence and toxicity checks on active therapy.
• Review chemotherapy orders for select providers prior to infusion.
• Assist with drug access and participate in CDTM practices as needed.
• Document care activities in the EHR and communicate changes to staff.
