Job summary

OhioHealth is seeking an Oncology Clinical Pharmacist to support both inpatient and outpatient care within the Bone Marrow Transplant-Hematology Program. The role includes direct patient care, formulary management, policy development, and quality improvement within a multidisciplinary team.

Primary duties involve optimizing oncology and supportive medication therapy, modifying regimens in the EHR, and ensuring age- and status-appropriate plans.

Qualifications

• BS or PharmD degree from an accredited institution.
• Current Ohio pharmacist license.
• BCOP or eligible for examination within the first 24 months of employment.
• BCOP certification must be maintained during length of employment in position.
• Completion of a Pharmacy Practice residency or equivalent hospital experience.
• Pharmacokinetic drug dosing experience.

Responsibilities

• Promote evidence- and value-based oncology care within the oncology care team.
• Recommend modifications to oncology/ supportive therapies based on patient data.
• Modify chemotherapy plans in the EHR with the multidisciplinary team.
• Document pharmacist interventions and discuss with providers.
• Counsel on medication adherence and provide education and motivational interviewing as appropriate.
• Understand PBMs and formulary requirements; identify alternatives for non-formulary drugs.
• Provide experiential instruction to Doctor of Pharmacy students.

Pharmacokinetic dosing experience

Education
