Collaborating with an interdisciplinary healthcare team, the nurse assesses needs, applies the nursing process, and implements and evaluates the care plan while educating patients and families for optimal health outcomes.

Responsibilities

• Delivers patient care by applying nursing process based on theory, research, and organizational standards.
• Assesses and evaluates patient care needs; uses critical thinking in patient care management.
• Integrates assessment and intervention skills in nursing care delivery.
• Collaborates with the interdisciplinary healthcare team in developing, implementing, and evaluating the plan of care.
• Prioritizes all aspects of patient care including teaching, rounding, and planning after hospital care.
• Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention.
