• Assess and evaluate patient needs and apply critical thinking in care plans.
• Collaborate with interdisciplinary team to develop and evaluate plans of care.
• Educate patients and families about plan of care, transitions, and health promotion.
• Demonstrate effective communication with patients, families and care team.
• Perform charge duties as assigned and ensure resources support patient care.
