Job summary

Allina Health is seeking an Oncology Nurse Navigator to coordinate care for cancer patients between the hospital and outpatient settings. This role supports GI cancers and may involve virtual responsibilities for non-Coon Rapids locations.

The incumbent will ensure seamless navigation across the care continuum, working with a multidisciplinary team. The position is 1.0 FTE with 3–5 onsite days weekly in a hybrid model, offering a meaningful scope in cancer care delivery and patient advocacy.

Qualifications

• Bachelor’s degree in nursing is required.
• 2+ years of inpatient or outpatient oncology nursing experience is required.
• RN licensure in MN is required upon hire.

Responsibilities

• Care Coordination and Navigation: provide care management and an integrated plan of care for oncology patients.
• Assessment, Planning, Implementation, and Evaluation: collaborate with oncology care providers and care team to develop and evaluate care plans.
• Team Participation and Quality Improvement: engage in quality initiatives and align with national oncology standards.
• Other duties as assigned.
