Saint Francis Hospital, Inc. in Tulsa, Oklahoma, is seeking an oncology nurse navigator to coordinate care across the hematology/oncology continuum, facilitate timely appointments, and connect patients with community resources.

This role serves as a liaison among physicians and the care team to improve patient outcomes and experience. Responsibilities include collaboration with multidisciplinary teams, patient education, documentation, and ongoing support from diagnosis through survivorship.

Qualifications

• Minimum education: nursing degree or diploma approved by a state board of nursing.
• RN license is required or obtainable.
• 3–4 years of related experience; oncology nursing or patient education preferred.

Responsabilités

• Participates in multidisciplinary tumor boards/conferences to facilitate care plans and expedite diagnosis and treatment.
• Establishes patient relationships, assesses understanding of the care plan, and supports adherence.
• Maintains accurate documentation and uses referral channels to communicate with the care team.
• Provides continuity of care from pre-diagnosis through survivorship, coordinating appointments and being available for patient questions.
• Reinforces education on disease and treatment to patients and families.
• Contributes to quality services by timely, cost-effective care and service line participation.
• Participates in Oncology Service Line and Cancer Committee activities.
• Attends community health fairs to educate the public about cancer prevention and care.
