Job summary

Saint Joseph Health System in Mishawaka, IN seeks an Oncology Patient Navigator to assist physicians and disease-site teams from diagnosis through treatment and recovery, coordinating care and linking patients to medical center and community resources.

The role includes patient and staff education, program facilitation, data collection, and ensuring smooth flow of care across services to improve quality outcomes for cancer patients.

Qualifications

• Bachelor of Nursing Degree preferred; Master's degree preferred.
• RN Indiana license preferred.
• 3+ years acute clinical experience; oncology experience preferred; discharge planning or case management acceptable; Spanish preferred.

Responsibilities

• Coordinate oncology care from diagnosis through treatment and recovery.
• Educate patients, families, and staff; develop materials and resources.
