Stony Brook Medicine is seeking an Oncology Nurse Coordinator to work with physicians and nurse navigators to streamline care for cancer patients.

The role focuses on establishing care maps, coordinating treatments, and serving as a patient resource throughout the treatment journey, in collaboration with attending staff and nurses. Related experience in oncology and BSN completion are preferred for this position.

Competenze

NYS license to practice as a registered nurse in New York State.
Minimum of 3 years of acute nursing experience, with at least 2 years in oncology.
Bachelor’s degree in Nursing or current enrollment in a BSN program (proof required).
Preferred: Certified Oncology Nurse.
Experience coordinating outpatient cancer care is a plus.

Mansioni

Establish and coordinate a care map for patients undergoing active cancer treatment.
Serve as liaison and support system to ensure organized, timely treatment.
Provide education to patients and families about the treatment plan and care options.
Facilitate communication among physicians, nurses, residents, and staff.
Track and update plans of care addressing educational, financial, social work needs.
Schedule treatments and clinic visits to ensure timely delivery of care.
Develop and update policies related to care coordination and survivorship planning.
Assist in completion of cancer survivorship care plans with disease management team.
