Trinity Health IHA Medical Group in Michigan seeks a compassionate Care Manager - Oncology for outpatient care. You will guide patients through their healthcare journey with clinical triage, care coordination, patient education, and ongoing support that improves outcomes and patient experience.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan with CPR certification.
• Prefer CCM/CCP certification.

Responsibilities

• Active member of the office based care team.
• Provides care management, care coordination and navigation for patient beginning chemotherapy.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.

What We're Looking For

• EDUCATION: Graduate of an accredited Registered Nurse program.
• CREDENTIALS/LICENSURE: Valid, unrestricted RN license in the State of Michigan. Valid CPR certification. Prefer CCM/CCP certification.
• MINIMUM EXPERIENCE: 3-5 years of clinical experience with oncology patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital, within the past five years: knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education.
