Trinity Health IHA Medical Group seeks a compassionate Care Manager - Oncology to join our outpatient care team. In this role, you'll guide patients through their cancer journey with clinical triage, care coordination, patient education, and ongoing support to improve outcomes.

You will collaborate with providers, pharmacists, behavioral health specialists, and the care team; require an RN license in Michigan and 3–5 years of oncology experience; hybrid schedule includes two days in Brighton,

Competenze

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in Michigan and CPR certification; CCM/CCP preferred.
• 3–5 years of oncology patient care experience in ambulatory or inpatient settings in the last five years.

Mansioni

• Active member of the office based care team.
• Provides care management, care coordination and navigation for patient beginning chemotherapy.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
