CHRISTUS St. Vincent Health System in Santa Fe, NM is seeking a Registered Nurse to manage and coordinate Care Coordination services across ambulatory and inpatient settings. You will partner with clinicians to develop pathways, respect patient choices, and promote person-centered care.

The role emphasizes collaboration with an interdisciplinary team, use of electronic medical records, and building community referrals to support continuous quality outcomes. Per diem/varied shifts available.

Qualifications

• Registered Nurse with BSN preferred.
• Minimum two years of healthcare experience.
• Experience with quality, care coordination and population health preferred.

Responsibilities

• Provides Care Coordination services in the ambulatory setting that supports quality patient care across the continuum.
• Ability to work in a variety of electronic medical records systems and compile patient information into a single file; demonstrates strong critical thinking.
• Maintains coordination of health care for patients within CSVCG; interacts with physicians, administrators, managers, care coordinators and patients to ensure integrated care.
• Communicates effectively to promote harmonious relationships across care settings.
• Develops relationships with community resources (SNF, LTAC, home health, hospice, palliative care) and maintains knowledge of services.
• collaborates with the interdisciplinary team to ensure referrals, follow-up and optimal patient outcomes; closes the loop on referrals.
• Exhibits behaviors that promote patient satisfaction, respect for patient rights and confidentiality.
• Participates and can lead interdisciplinary groups; tracks and analyzes patient satisfaction and outcomes.
