Job summary

Qualifications

• Registered Nurse with active licensure
• BSN preferred for nursing leadership and care coordination
• Minimum two years in health care setting with care coordination experience
• Understanding of community health services and patient advocacy

Responsibilities

• Provide care coordination in ambulatory and clinic settings
• Compile patient records from various EMR systems into a single file
• Coordinate referrals to SNF, LTAC, home health, hospice and related services
• Collaborate with physicians, managers and staff to ensure integrated care
• Develop relationships with community resources to support continuity of care
• Communicate effectively to maintain patient satisfaction and confidentiality
• Lead interdisciplinary teams and target improved patient outcomes
• Track patient satisfaction and outcome measures
• Close the loop on referrals and follow-up care
