HCA HealthONE Presbyterian St. Luke’s in Denver seeks an Oncology BMT Social Worker III (LCSW) to support inpatient and outpatient oncology patients through transplant, survivorship and discharge planning.

You will work with a multi-disciplinary team to assess psychosocial needs, provide counseling, and assist with resources including SSDI/Medicaid/COBRA as appropriate. Qualified candidates hold an MSW and Colorado LCSW, with 3–5 years of healthcare social work experience; oncology experience

Qualifications

• Master of Social Work (MSW) from an accredited school of social work.
• Licensed Clinical Social Worker (LCSW) in the State of Colorado.
• 3–5 years of clinical social work experience in healthcare; oncology experience preferred.

Responsibilities

• Work with inpatient and outpatient multi-disciplinary oncology team to identify and meet needs of oncology patients as they progress from diagnosis through survivorship.
• Assess psychological, social, cultural and financial needs impacting patients through transplant.
• Provide counseling and clinical support for patients and families through the transplant process.
• Communicate effectively with providers, patients, families and care team; resolve conflicts and solve psychosocial issues.
• Conduct discharge planning assessments and referrals to post-acute facilities.
