Responsibilities

• Perform MTM in-person and telephonic pharmacotherapy consultations.
• Complete comprehensive medication reviews and assess interactions and adherence barriers.
• Provide pharmacotherapy assessments of medical history and disease states for optimized therapy.
• Deliver evidence-based therapeutic recommendations and educate patients on medications.
