Job summary

Qualifications

Responsibilities

• Provides education and information to patient and family.
• Relays information between providers and clients, incl. pharmacies and facilities.
• Schedules diagnostic, therapeutic, referral, or visits.
• Supports end-of-life care with emotional support.
• Represents a liaison for community educational events and health screenings.
