Hamilton Health Box (HHB) is a rapidly growing healthcare startup operating hybrid telemedicine-enabled primary care clinics for small and medium businesses across the country.

The On-Site Clinical Assistant (OSC) will perform daily clinical and administrative tasks at a local HHB location, including patient scheduling, following provider-directed care, and coordinating with pharmacy and referral networks to deliver high-quality, cost-effective care.

Formación

• Ability to communicate effectively at all levels throughout an organization.
• Ensure business is conducted in a timely, confidential, courteous, and professional manner.
• Knowledge of disease and lifestyle topics such as smoking cessation, weight management, nutrition, stress reduction and chronic conditions.

Responsabilidades

• Collaborate with other departments to ensure employees receive appropriate care and programs.
• Formulate individualized programs based on health risk assessment results while targeting high-need departments with programs and interventions.
• Assist with annual physicals and placement examinations, complete screenings, testing, and results reporting.
• Help schedule and complete follow-up exams, return-to-work exams, and consultations.
• Coordinate client referrals to other providers and document progress in medical records.
• Participate in health fairs, coordinate training sessions for regulatory requirements.
• Collect and interpret data on evaluations and utilization; document thoroughly.
• Provide one-on-one counseling and interpret screening results for clients.
