Job summary

You will ensure accurate documentation to support care planning and reimbursement, collaborate with interdisciplinary teams, and stay current with regulatory changes impacting MDS and skilled nursing documentation.

Required: prior MDS experience in long-term care, an active LVN/LPN license, and proficiency

Qualifications

• Current LVN/LPN license in the state of practice.
• Prior MDS 3.0/RAI experience in a skilled nursing or long-term care setting.
• Knowledge of CMS/Medicaid reimbursement guidelines.
• Proficient with electronic health records (EHR) systems.

Responsibilities

• Complete MDS assessments within required timelines (MDS 3.0/RAI).
• Ensure accuracy of resident documentation to support care planning and reimbursement.
• Maintain CMS/state/facility guideline compliance.
• Collaborate with interdisciplinary team to develop individualized care plans.
• Monitor resident progress and update care plans as needed.
• Participate in audits/surveys with accurate documentation.
