Responsibilities

• Maintain ongoing communication with providers through reports and multidisciplinary meetings.
• Assist patients in scheduling preventive screenings and follow-up appointments.
• Provide health education and counseling using standardized protocols.
• Serve as a resource in care transitions and quality metrics for staff.
• Demonstrate leadership in a high-performance clinical environment.
