This on‑site LTC pharmacist role focuses on delivering high‑quality patient care within a closed‑door pharmacy, collaborating with nurses, facilities, and providers to optimize therapy and ensure compliance with state and federal regulations.

Qualifications

• Doctor of Pharmacy (PharmD) and active Delaware license.
• Experience in long-term care pharmacy preferred.
• Excellent communication with nurses and providers.
• Detail-oriented with fast-paced LTC workflow.
• Knowledge of DEA/state regulations and HIPAA.

Responsibilities

• Verify orders entered by technicians for accuracy.
• Perform DURs, allergy checks, drug interactions, and duplication reviews.
• Review MARs and facility orders for clarity and compliance.
• Provide clinical recommendations to care teams.
• Participate in care plans, audits, and quality initiatives.
• Maintain security and compliance with regulations.
