Job summary

Chesapeake Regional Rx in Wilmington, DE seeks a Pharmacist to join its LTC-focused team in a non-retail setting. You’ll perform medication verification, clinical review, and ensure accuracy prior to dispense in a modern dispensing environment.

Key duties include DUR, drug interaction checks, and providing clinical recommendations to nurses and facility staff, with an alternating biweekly schedule and a 3-day weekend every other week.

Qualifications

• PharmD or active license required; LTC background a plus.
• Active Delaware Pharmacist License (verified before start).
• Experience with a pharmacy management system.
• Strong communication across nursing staff, facilities, and providers.
• Able to work efficiently in a fast-paced LTC dispensing environment.

Responsibilities

• Verify technician-entered orders for accuracy against MAR and provider orders.
• Conduct DUR, allergy checks, drug interaction screening, and duplication checks.
• Review eMAR entries, provider notes, and facility orders for clarity and compliance.
• Check final packaging and labeling for accuracy prior to dispense.
• Provide clinical recommendations to prescribers, nurses, and facility staff.
• Review PRN and routine regimens for safety and appropriateness.
• Assist with medication regimen reviews and consulting support as needed.
