Qualifications

• Master’s level degree in psychology or related field, Ph.D. or PsyD from an accredited college or university.
• Licensure to practice in Indiana: LCAC, LMFT, LMHC or LCSW.
• CPI and BLS certifications obtained after hire and kept current.
• Valid driver’s license and active auto insurance.
• Satisfactory results from criminal and driving record checks.

Responsibilities

• Assess patients and diagnose mental health and/or substance use disorders.
• Create a treatment plan for patients based on individual needs.
• Refer patients to other healthcare professionals when needed.
• Provide counseling services using evidence-based practices within scope.
• Maintain credentials with managed care contracts and complete documentation.

Education

Master’s level degree in psychology or related field
