Job summary

Pair Team is seeking a Field Hospital Enrollment Specialist to serve as on-site presence within hospital emergency departments and inpatient units in California. You will identify eligible Medi-Cal/Medicare patients, educate them about Enhanced Care Management, and support enrollment into Pair Team’s care navigation program before or shortly after discharge.

This patient-facing role collaborates with social workers, case managers, nurses, and registration teams to create a smooth enrollment

Qualifications

• Bilingual fluency in English and Spanish is required.
• 1+ year of experience in patient outreach, community health, enrollment, patient navigation, care coordination, or a related healthcare setting.
• Previous hospital/ED/inpatient experience preferred.
• Experience communicating with diverse patient populations.
• Ability to explain healthcare programs clearly and build trust.
• Strong organization, follow-through, and documentation skills.

Responsibilities

• Review daily patient census reports with hospital teams to identify eligible Medi-Cal/Medicare patients.
• Conduct in-person bedside outreach in EDs and inpatient units.
• Verify patient eligibility, health plan, and enrollment status.
• Educate patients and families about Pair Team’s Enhanced Care Management program.
• Enroll eligible and consenting patients before discharge when possible.
• Document outreach, patient status, attestations, and enrollment activity in the EMR.
• Coordinate warm handoffs and communicate follow-up needs to Pair Team and hospital partners.
• Participate in shift huddles and hospital workflow meetings as needed.
• Build strong relationships with hospital staff across units.
