Job summary

Qualifications

• Current, unrestricted Utah Pharmacist license.
• Doctor of Pharmacy (PharmD) or equivalent degree.
• Strong clinical pharmacy skills including MTM, drug interaction review, and patient counseling.
• Experience or interest in community health, primary care, and SUD treatment.
• Proficiency with pharmacy management systems and EHRs.
• Excellent communication in a multidisciplinary team.
• Commitment to equitable, patient-centered care.
