Job summary

Neighborhood Health Center in Portland, OR seeks a Behavioral Health Consultant to provide expert integrated care across adult and pediatric patients within our primary care teams.

The role emphasizes brief, evidence-based assessments with a focus on functional improvement, collaboration with physicians, and high-volume, team-based care. Spanish fluency is highly preferred and EHR (Epic) documentation is essential.

Qualifications

• Master’s degree in a mental health-related field (Counseling, Psychology, Social Work) or PsyD is required.
• License in Oregon (LPC, LCSW, LMFT) is required.
• Clinical experience in integrated care settings preferred.

Responsibilities

• Provide expert integrated behavioral health care for adults and pediatrics within primary care.
• Conduct brief, evidence-based assessments and interventions for mental health and substance use concerns.
• Collaborate with PCPs and care teams; communicate treatment plans concisely.
• Use PHQ-9 and Columbia Risk Assessment for risk screening; manage safety planning when needed.
• Document during sessions and support billing and compliance requirements.
• Offer training and consultation to primary care staff on behavioral health topics.
