Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Active, unencumbered license in the state of employment.
• Basic computer skills and ability to navigate EHR and service delivery software.

Responsibilities

• Return patients to optimal physical function in SNF settings.
• Educate patients to maintain function after discharge through education and independent exercise plans.
• Collaborate with therapists and physicians to achieve patient goals.
